Our Medina Location
Our office is in the 620 E. Smith Industrial Park area. Enter the double driveway off 620 E. Smith Rd., turn left and our Unit E-5A is located in the middle of a one-story strip of offices on your immediate left. Parking is available. See our interactive map below.
Our Lodi Location
Our Lodi office is at 225 Elyria St., Lodi. We are in suite 103 behind the Lodi Hospital ambulatory and emergency areas in an administrative building.
